Service Coordinator

Location: Albany, NY

Schedule: Monday – Friday, 8:30am – 5:00pm

Position Description: Our Service Coordination department is looking to add a Service Coordinator to it's team.  Service Coordinators offer support to individuals living in a variety of settings within the community, and assist individuals/families in accessing appropriate programs, benefits, and other opportunities which improve quality of life.

Qualifications:

  • The ability to work independently as well as in a team environment; with flexibility to meet program needs
  • A bachelor’s degree in either health or human services field with a minimum of one year working with people with a developmental disability or one year working as a Service Coordinator with any population.  **A bachelor’s degree in other fields will be considered with proof of a minimum 20 credits in human services related courses
  • Valid NYS driver’s license and personal, reliable vehicle
  • Physically able to perform job duties
