Community Supports Supervisor/Support Broker

Schedule: Monday – Friday 8:30am – 5:00pm, some flexibility required

Position Description: Our Individualized Community Services Department is seeking a dedicated professional to join its leadership team.  The ideal candidate is a positive, detail oriented, team player with strong communication skills. 

The Community Supports Supervisor/Support Broker is responsible for directing the day-to-day operational requirements associated with providing direct care to individuals with developmental disabilities living in the community.  This position will also assist individuals in our Self Direction program to develop a comprehensive Self-Directed Budget that is consistent with his/her Individualized Service Plan (ISP) and work in tandem with the individual and planning team to ensure that all necessary safeguards are included and addressed in the ISP.

Other job responsibilities include:

  • Management and oversight of a team of approximately 20 staff
  • Collection, review and submission of Medicaid billing and payroll documentation.
  • Ensure all Medicaid and OPWDD regulatory requirements are met.
  • Maintain regular contact with individuals/families receiving services through CCDS
  • Assist the individual(s) to develop and maintain a planning team/circle of support and assisting in directing planning meetings.
  • Assist the individual(s) to develop a comprehensive Self-Directed Budget and habilitation plan that is consistent with his/her Individualized Service Plan.

Qualifications:

  • A Bachelor’s degree in Human Services or a related field is required. 

  • Applicants must have at least one year of experience working with individuals with developmental disabilities; supervisory experience is preferred. Support Broker training preferred.

  • A valid NYS driver’s license in good standing and personal transportation are also required, as this position requires regular travel within the Capital District.
