TBI Service Coordinator Location: Albany, NY Schedule: Monday – Friday, 8:30am – 5:00pm, Some flexibility and on-call phone coverage is required Position Description: The Individualized Community Services department is seeking a TBI Service Coordinator to join their team. TBI Service Coordinators offer support to individuals living in a variety of settings within the community, and assist individuals/families in accessing appropriate programs, benefits, and other opportunities which improve quality of life. Qualifications: Candidates must be able to work independently and in a team environment, and be flexible to meet program needs. A Bachelor’s or Master’s Degree in a Health or Human Services field is required. Candidates with a Bachelor’s Degree, must have, at minimum, three years of case management experience providing: case management/service coordination, information and referral services and service brokering of community based services for individuals with disabilities and/or Traumatic Brain Injury. Candidates with a Master’s Degree must have, at minimum, one year of aforementioned case management experience.
