Person Centered Excellence Report, November 2015

November proved to be a busy and productive month on our journey toward Person-Centered Excellence.

The Operations Committee met to discuss how we will monitor our compliance with Basic Assurances. We agreed to use a dashboard model to report on metrics measured across the nine Basic Assurance factors, utilizing one or two metrics for each factor. We will decide upon the metrics for factors 1, 2, and 3 in December, 4, 5, and 6 in January, and 7, 8, and 9 in February. The Operations Committee also discussed how to use Life Journey Conversations to drive service planning for the folks who live in our ICFs during the conversion to IRAs. Finally, we discussed the possibility of changing the process in which these conversations take place.

New agency standards (policies) were issued in November defining our practices around Person-Centered Planning, conducting Life Journey Conversations, and garnering input from the individuals we support in the hiring of staff.

We also hosted another CQL Community of Practice meeting. Attending were representatives from all seven accredited agencies, as well as both voluntary and state programs thinking about pursuing accreditation.

Finally, we met with a sister agency who is starting the process of accreditation to share some of our experiences.
